This will be used to register Microchip. Type accurately.
Spouse, Boyfriend, Parent or person living with you and the cat.
You must be 21 to adopt. A driver’s license or ID is required to adopt.

Household Information

Please list members of house hold and ages.

Meaning do you have family or friends children that will be around the cat with regularity due to babysitting, neighbors kids, etc.

Current Pet Information

List all the pets you have owned in past three years.

Please list the vet your pets have seen for vaccinations.

I understand that WAHS only adopts cats to live indoors in a home. We do not adopt cats out for gifting to others. We do not allow declawing of cats adopted out. I know I must provide a vet contact for my current animals. I am aware that if I rent my landlord must approve of me having a pet. I am signing below in agreement that everything I said in this application is true to the best of my knowledge and that falsifying information will lead to the cancellation of the application.

Submitting an application does not guarantee you will adopt the cat you want. The WAHS’s goal is the find the best fit for the cat. Adoptions are not first come first serve.

Have these items ready:

  • Names of cats you want to meet
  • List of current household members
  • List of current household pets
  • Make sure all current pets in household are up-to-date on rabies
  • Current veterinarian & office phone number

Be Aware:

  • The WAHS does not adopt to anyone under 21
  • We do not allow declawing of our adopted cats
  • We only adopt cats to be indoor pets in a home
  • We don’t allow cat adoptions for gifting to another
  • Adoptions are not first com first serve
